What Actually Happens at a Hygiene Appointment (And Why It's the One Most People Skip)

Plenty of people book a check-up but quietly let the hygiene appointment slide. It feels optional. A bit of a luxury, even. Something you can put off until things settle down.
Here’s the problem with that thinking: the tells you what’s wrong, but the hygiene appointment is often where the prevention actually happens.
So if it’s been a while — or if you’ve never been entirely sure what a hygienist does that you can’t manage at home with a good toothbrush — this one’s for you.
The phrase "scale and polish" undersells the whole thing. Yes, there’s a polish at the end. But the bulk of the appointment is about removing what your toothbrush physically cannot.
When plaque sits on your teeth long enough, it hardens into tartar (sometimes called calculus). Once that happens, brushing won’t shift it — no matter how diligent you are or how expensive your electric toothbrush was. It bonds to the tooth and creeps below the gumline, where gather and start to the gum. Removing that, safely, in the places you can’t see or reach, is what a hygienist is trained for.
So if you brush well and still get told you have build-up, you’re not doing anything wrong. Some tartar is normal. That’s exactly what the appointment is for.
This is the question that keeps a lot of people away, and it’s a fair one. The old image — scraping, scratching, tender gums for the rest of the day — puts people off for years at a time.

Left unchecked, that inflammation can progress to the point where it affects the bone holding your teeth in place. Gum disease is the leading cause of tooth loss in adults — ahead of decay. There’s also growing linking gum health to wider health, including the heart and the management of conditions like diabetes.
A hygiene appointment catches the early signs before they turn into painful, expensive problems. And if something more is needed, Dr Shivani Patel — who holds a Master’s degree with Distinction in periodontology — has a dedicated focus on gum health and takes the time to show patients how to look after it properly at home.
There’s no single answer, which is why people get confused. If your gums are healthy and your habits are good, every six months alongside your dentist is usually fine. If you’re prone to build-up, have early gum issues, smoke, or are going through orthodontic treatment, every three to four months tends to suit better.
The honest version: your hygienist will tell you what your mouth needs rather than sell you a schedule. Most people land somewhere sensible once they’ve had one proper clean and can feel the difference.
If you’re the one keeping everyone’s appointments in the family diary, build the hygiene visit in early — for the children and teenagers too. Habits formed young save a great deal of trouble later, and a hygienist is often far better placed than a parent to explain why flossing matters to a sceptical fourteen-year-old.
